  • Administrator-led by a Doctor of Nursing Practice with a Registered Nurse Back-Up Operator — ensuring resident health is actively monitored and medication is overseen with clinical precision.

  • Individualized behavior management grounded in Positive Behavior Support, de-escalation, and trauma-informed communication that honors the person behind the behavior.

  • Daily routines that invite participation in meal preparation, household tasks, personal care, and community navigation — building confidence at each resident's own pace.

  • Real connections within Spring Hill — recreational, cultural, religious, and volunteer participation. Belonging is treated as a health outcome, not an optional activity.

  • Nutrition, physical activity, rest, and preventive care embedded into daily life through a dietitian-approved menu and structured environmental wellness practices.

Meet the Founder

Care Smith was founded by Suzanne Levy, DNP, AGPCNP-BC, ONC, a doctoral-prepared, board-certified Adult-Gerontology Nurse Practitioner with 14 years of healthcare experience. Throughout her career, she has cared for adults with diverse medical, behavioral, and support needs across a range of clinical settings.

Her dedication to serving individuals with developmental disabilities began early in her healthcare journey while working as a Direct Support Professional in a residential group home in Yonkers, New York. There, she witnessed firsthand how compassionate support, meaningful relationships, and community inclusion can empower people to build confidence, gain independence, and lead fulfilling lives.

Inspired by the belief that every person deserves the opportunity to thrive, Dr. Levy created Care Smith, a nurse-owned and operated residential group home dedicated to wellness, inclusion, and personal growth. Through the CareWELL™ Program, residents build meaningful connections, engage with their community, and pursue lives filled with purpose and possibility.
